What is Interwell Medical Group, and how can it help me?

Interwell Medical Group provides telehealth visits for people with chronic kidney disease (CKD) stages 3–5 who need additional medical support beyond their existing healthcare. Our team helps you manage your condition through video or phone calls and connects you with a local kidney doctor for long-term care.

 

What is chronic kidney disease (CKD), and how do I know if I have it?

Chronic kidney disease happens when your kidneys don’t work as well as they should over time. It often starts without any symptoms and late-stage symptoms such as swelling, fatigue, reduced appetite, and increased blood pressure may easily be overlooked or confused with other conditions. CKD is detected through commonly performed blood and urine tests obtained by your doctor. CKD can affect your overall health, so taking steps to manage it early is important.

 

What stages of chronic kidney disease do you treat, and what does that mean for me?

We care for people with stage 3–late stage 5 chronic kidney disease. This means your kidneys aren’t working as well as they should and getting help early can prevent more damage. We’ll create a care plan that works for you and guide you to local resources if needed.

 

What makes Interwell Medical Group different from other kidney care providers?

We help fill gaps in care. If you don’t have access to a kidney doctor, our nurse practitioners, who specialize in kidney care, provide virtual care to support you. Our team uses technology to offer personalized, caring support to meet your needs.

 

What is the difference between Interwell Medical Group and Interwell 360?

Interwell Medical Group provides virtual care from our expert nurse practitioners when you need additional medical support. Interwell 360 is a benefit that provides ongoing telephonic care management to support your kidney health and lifestyle needs. Services and care

How does virtual kidney care work?

Virtual care means you meet with our kidney experts over video or phone. We’ll help you manage medications, understand lab results, and create a care plan.

 

What kind of support can I expect during a virtual visit?

During your virtual visit, our team will:

  • Guide you on your medications
  • Order any tests you may need and help you understand the results
  • Work closely with your regular doctor if needed
  • Take steps to connect you with a local nephrologist

 

Who will I speak to during my virtual visit?

You will speak with a nurse practitioner on our expert care team.

 

How do you work with my other healthcare providers?

We team up with your other doctors to make sure your care is seamless. We share important information, like test results and care plans, to keep everyone on the same page.

 

What happens if I already have a kidney doctor?

If you already have a kidney doctor, they will continue to manage your ongoing kidney care. However, if you have recently been in the hospital and your doctor is not immediately available, we are available to support you with a virtual follow-up appointment to bridge your care until you are able to see your primary nephrologist.

 

What happens if I just got out of the hospital?

If you've just been in the hospital, we are here to help with follow-up care. We’ll check your medications, see how you’re doing, and help you find more care if you need it.

 

How long will I get care from Interwell Medical Group before switching to a local kidney doctor?

We aim to connect you with a local kidney doctor as soon as possible. Think of us as temporary support until you’re set up with long-term care.

 

Technology and appointments

What do I need for virtual visits?

You’ll need a smartphone, tablet, or computer with a good internet connection for video appointments. If you don’t have internet, we can do phone appointments instead.

 

How can I schedule an appointment?

Scheduling is easy. Just call us at 1-833-664-8244. We’re happy to help.

 

What should I do if I have a medical emergency?

Call 911 right away if it’s an emergency. For non-urgent issues, contact us during regular hours at 1-833-664-8244.

 

Is my health insurance going to cover your services?

Many health insurance plans cover our services. Check with your health plan to confirm your coverage.

 

What is the cost to me?

Depending on your health insurance plan, you may have a co-pay and/or other costs. Check with your health plan to ask about your coverage.

 

Do I have to join this program?

No, joining our program is your choice. You can leave anytime without affecting your current health plan or doctor. As a part of your health plan, you will still receive Interwell 360 benefits, even if you elect not to receive benefits from Interwell Medical Group.

 

Does getting virtual care through Interwell Medical Group affect my health plan or benefits?

No, Interwell Medical Group does not affect your current health plan and benefits.

 

Is my personal health information secure?

Yes, protecting your privacy is our priority. With your consent, Interwell Medical Group care team members use your health information to create the right care plan for you. Your personal health information is handled in compliance with strict privacy laws.

 

What if I have questions about my medications or test results?
